Release and Waiver of Liability

By registering for this event online and in person, you are agreeing to release Buckeye Broadband, the Ovarian Cancer Connection, and the location/facility of the event & vendors, from liability and have therefore read it carefully. I understand that this event involves physical activity, and I agree that I am physically and medically able to participate in the event. I also agree to immediately stop participating and request assistance if I experience any condition which may make it unsafe for me to continue. I also understand that my participation in this event is voluntary and involves the risk of injury and property damage, which I assume. I agree to release Buckeye Broadband and the Ovarian Cancer Connection and any other event sponsor including each of their affiliates, employees, agents, and volunteers from all liability arising out of my participation in this event and to hold them harmless from all liability arising from my actions while participating in this event. I agree that any photographs, videotapes, drones, recording of any kind other than images containing my likeness (of the Minor's) are sole property of the OCC or the photographer. I hereby authorize Buckeye Broadband and the Ovarian Cancer Connection, its agents, sponsors, and/or news organizations to use or publish said images for news accounts, publicity, or promotional purposes for any other lawful purpose through any form of media whatsoever .Additionally, in consideration of my voluntary participation in an event/activity known as The Ellen Jackson Ovarian Cancer Walk, and taking place on the Buckeye Broadband, I on behalf of myself, my heirs, executors, administrators and assigns, do hereby release and forever waive any and all claims against Buckeye and the Ovarian Cancer Connection, its affiliated companies, all Event sponsors and promoters, and all public agencies; and all of employees, officers, directors, shareholders, affiliates, agents and volunteers of any of the foregoing; as well as their successors and assigns; for any and all liability and claims for injury or death arising from my participation in the Event, including any acts of negligence on their part. The foregoing includes without limitation death or injury arising from my participation in the Event carries with it the risk of physical injury and even death. I am voluntarily participating in the Event with knowledge of the danger and risk involved. I hereby agree to accept and assume all risks of injury or death by my participation. I certify that I am an adult over the age of eighteen (18) and competent to sign this release and Waiver of Liability. I will abide by all directors on all matters and that shall be final. I grant Buckeye and its affiliates permission to record my voice and image in conjunction with the Event; grant full unrestricted rights to use of my name, image, likeness, and voice in any form, in or over any medium including print, internet, broadcast, cable, satellite transmissions, and any other media for any purpose including a commercial
purpose in perpetuity, for no consideration now or in the future, I have carefully read this Release and Waiver of Liability and | am signing it of my own free will.
